Aflac Accident Insurance
Employees that wish to cancel their coverage may do so by logging in to the Nevada PEBP+ Voluntary Benefits site through your E-PEBP portal; going to the Aflac Accident Insurance page; clicking the enrollment link; and choosing "Cancel." Employees may also contact customer service at 1-855-901-1100.
Aflac Critical Illness Insurance
Employees that wish to cancel their coverage may do so by logging in to the Nevada PEBP+ Voluntary Benefits site through your E-PEBP portal; going to the Aflac Critical Illness Insurance page; clicking the enrollment link; and choosing "Cancel." Employees may also contact customer service at 1-855-901-1100.
Aflac Hospital Indemnity Insurance
Employees that wish to cancel their coverage may do so by logging in to the Nevada PEBP+ Voluntary Benefits site through your E-PEBP portal; going to the Aflac Hospital Indemnity Insurance page; clicking the enrollment link; and choosing "Cancel." Employees may also contact customer service at 1-855-901-1100.
ASPCA Pet Health Insurance
To cancel you will need to contact ASPCA directly at 1-877-343-5314.
ID Watchdog Identity Theft Protection
Employees that wish to cancel their coverage may do so by logging in to the Nevada PEBP+ Voluntary Benefits site through your E-PEBP portal; going to the ID Watchdog page; clicking the enrollment link; and choosing "Cancel." Employees may also contact customer service at 1-855-901-1100.
LegalEASE Legal Plan
Employees that wish to cancel their coverage may do so by logging in to the Nevada PEBP+ Voluntary Benefits site through your E-PEBP portal; going to the LegalEASE page; clicking the enrollment link; and choosing "Cancel." Employees may also contact customer service at 1-855-901-1100.
Liberty Auto and Home Insurance
To make any changes to your policies, including cancellations or changes to payment options, please contact Liberty Mutual directly at 1-800-298-8018 or call a local office which you can find at www.libertymutual.com. Please reference your policy number, which can be found in your insurance policy. States have different requirements based on DOI regulations.
MetLife Auto and Home Insurance
Cancellation policies differ by state; therefore, the employee must call the MetLife Call Center at 1-800-438-6388 for cancellation terms and processing.
Nationwide Pet Health Insurance
Policyholders may cancel a policy at any time by contacting Nationwide directly at 1-877-738-7874 or optionally, by notifying Nationwide in writing via fax at 1-714-989-0537, or mail at P.O. Box 2344, Brea, CA 92822, or online at the
Policyholder Portal.
Travelers Auto and Home Insurance
To cancel a Travelers Insurance policy, the employee needs to call the Travelers Customer Service Center at 1-888-695-4640. Please reference your policy number, which can be found in your Insurance policy. If the policy has been paid in full or is on direct bill, any refund on earned premium due to the employee will be sent to them. In the case of payroll deduction, Travelers may need to take one or more final deductions until the policy is paid back to the cancel date since payroll deduction policies are billed in arrears.
VSP Vision Care
The vision plan is a year-long commitment. Employees that wish to cancel their coverage may do so only during annual open enrollment by logging in to the Nevada PEBP+ Voluntary Benefits site through your E-PEBP portal; going to the Vision Care page; clicking the enrollment link; and choosing "Cancel." Employees may also contact customer service at 1-855-901-1100.
